I've been messing with a few programs and http://www.swift-tools.net/Quake/QVM/ is a neat program that lets you put music into your movie and you can try different encoding codecs too. I have experienced this problem while watching RTCW demos played back at high speed for a few seconds. The audio will stop being synchronized with the demo once you return to 1.0 speed.
